Step 4:

When you click on your lead form extension a popup will open where you have to accept the terms of service and create a new form.

After you have accepted the term of service, you can create your own lead form.

Step 1:

The people viewing your form will be standard text, such as title, business name, description. Here you can also get audience information
such as the name of your audience, the phone number of the audience and the city they bill from This is how you can take many fields according to your need.

Step 2:

Create a message to submit the form. Create Heading for it, Create Description, Create URL for Call to Action.
